Insurance Plans
Your insurance coverage is an agreement between you and your insurance company. Financial responsibility rests with the patient for deductibles, co-insurances, and non-covered services. Insurance coverage is a variable and cannot guarantee what services will be covered by your particular plan. If you have an HMO type of insurance plan, then you will need to get a referral from your family doctor or primary care provider. It is your responsibility to know the requirements of your insurance program. We will ask you to sign an authorization form requesting that insurance payments be sent directly to our office when appropriate.
We accept most insurance plans with a few exceptions, and we are currently in the process of getting Dr. Paz credentialed with more insurance plans. Please call our office to inquire if you have any questions.
Co-Payments And Deductibles
Co-payments and deductibles are due at the time of service. For managed care patients, applicable co-payments and deductibles will be collected. We asked that you pay any unmet deductible, non-covered service and co-payments at the time of service.
For your convenience, we accept personal checks, cash, and all major credits cards.

Nonpayment And Returned Checks
Nonpayment of your account balance will result in collections. A 0.00 fee will be added to your account for returned checks.
Cosmetic Consults
Patients that have a cosmetic consult will receive credit in the amount of the consult toward their cosmetic procedure, if scheduled and performed within six months.
Cancellation Policy
Please provide at least 48 hours notice if cancelling surgery to allow another patient an opportunity to fill your surgery date. We request 24 hours notice if you need to reschedule a cosmetic, general dermatology, follow up, or full skin exam appointment.
Pathology And Labs
Biopsies may be sent to a lab to be analyzed. The fee for this is a separate charge from the office visit and may be billed directly by the processing lab. Results are usually available 7-10 days after the test has been performed. Questions regarding payments for laboratory services should be discussed with the lab.
